The following
programs will be offered to members of the cosmic zone complimentary health club
Health Improvement Promotion Program (CHIPS): “Ask your provider” web site. At this
site, the club member will be able to interact with a provider team member of
choice, whether it be a Medical Doctor, nutritionist, herbalist, nurse,
podiatrist, Chiropractor, exercise physiologist,Psychologist or others. All e
mails will be answered in a timely manner. Each club member will be given a pass
word and user name. Questions regarding heath, health care delivery methods,
medicine, alternative medicine, holistic medicine and integrative health will be
addressed. Personalized Health Management Programs will be designed, monitored
for outcome and statistically analyzed for individual and group effectiveness.
Over time, we hope to have the data needed to prove with a high degree of PEER
reviewable certainty, that the Health Approach that you have divinely crafted is
the right approach for you.
Our second product will be a Health Access Program . (CHAPS)This program will
involve members of the club having assess to a health provider that can offer
treatment for diseases via the internet. Each club member will have an internet
treatment site equipped with telehealth capabilities. Each club member will be
give a password and user name each year as a subscriber as in Health Promotion.

to perform multiple tasks while you engage in health improvement activities. You
can get prescriptions refilled and communicate with our Health providers by
e-mail. Motivation is provided by our staff and customers. Every one wishes you
success. You will be in the mist of other motivated people who are changing
their lifestyles also. When you come into the “Zone” your spirits will be
immediately uplifted.

Health protection is one of the fringe benefits that must be included in any
competitive employee benefit package. CHAPS offer such a protection plan. With
it innovative demand management components, it offers interactive prevention,
early detection, early intervention, and monitoring capabilities for health and
wellness. CHAPS with its Virtual (e- visit) system, improves access and
availability of health services while saving time and controlling costs.

The Programs maximum yearly out of pocket expense is $240. After spending $240, no more
charges will be assessed for any services of a routine nature rendered by the
center or it’s staff except those mentioned below.
Club Members who are in programs and other business associates who have
members in programs and their guess will be eligible to use the facility for
program related activities after hours. The following conditions apply:
An agreement will be signed within 24 hours prior to use along with a
fee of $210 for expenses.
The above rates will cover 4 hours .Additional hours of use will be pro rated.
